Joining requirements
Complete our joining form, include the following details and documents:
- ID copy of the main member
- ID or birth certificate copy of all the dependents
- Proof of residence
- Contact details
- 1st month’s premium
- Individuals over the age of 18 years are eligible to apply
Claim Procedure
Submit the following documents to any of our offices
- Certified copy of death certificate.
- Certified copy of ID or passport for the deceased.
- Notification of death form DHA 1663,(formerly BI 1663).
- ID and certified ID copy of the beneficiary.
- Proof of banking details (signed, dated and stamped bank statement not older than three months) for all beneficiaries/estate.
- In the event of accidental death, please provide a police declaration.
- Marriage certificate for spousal claim.
- Please report the death to your nearest Home Affairs office who will also provide you with the official death certificate.
- No joining fee required.
- 1 month waiting period (only valid till 31 May 2023) 6 month waiting period from the
- 1st June 2023
- In the event of accidental death, your beneficiaries get 100% payout, with no waiting period.
- We do not conjointly conduct funeral services with other funeral service providers.
- If no premiums are paid for two consecutive months, the policy will lapse on the third of the third month. Reinstated policies will be subject to the policy’s initial waiting period.
- Benefits will not be paid out in cash.
- Claims not made within 6 months after death will not be accepted and approved.
